I/we accept the place offered to our daughter at Diocesan School for Girls.

I/we agree to pay the non-refundable enrolment fee of $1,750.00

I/we agree to abide by the School's Terms and Conditions of Enrolment.

I/we accept responsibility for the payment of all fees, levies and charges incurred and agree to submit a direct debit form for this purpose.

I/we acknowledge the School requires twelve weeks' written notice of withdrawal, and understand that one quarter's annual tuition fee will be charged in lieu of notice, where this requirement has not been met. Please refer to the School's Terms and Conditions of Enrolment for further information.

I/we give the School permission to collect information and make credit enquiries about us and to do the same in relation to the collection of overdue amounts. We also authorise any third party to provide the School with the information as it may require for credit enquiries or for collection of overdue payments. We authorise the School to provide any third party with details it may have concerning us for credit rating or credit enquiry purposes.
